Privacy Policy

Last reviewed: not yet reviewed

This page has not been reviewed by a lawyer. What it describes below is accurate to how the website actually works, but it has not been checked for legal sufficiency in any jurisdiction. Do not launch a client site in this state.

Who we are

Bayshore Physical Therapy PLLC operates this website. If you have a question about your information, or want a copy of it or it deleted, write to frontdesk@bayshorept.example.

This is not our Notice of Privacy Practices

Two different documents cover two different things. Your medical record is protected health information under HIPAA, and how we handle it is described in our Notice of Privacy Practices, which is available at the front desk. This page covers something narrower: the contact form on this website.

The form is deliberately built so it cannot carry clinical information. There is no box to describe symptoms and no field asking why you are coming in, because that information would travel to a third-party form service. Please keep medical detail off this website and tell us by phone or at your visit instead.

What this site collects

Only what you type into the request form, and only when you submit it. Nothing is collected from you just for reading these pages.

  • your name
  • your phone number
  • your email address
  • the times you said you could be reached

This list is generated from the form itself rather than written by hand, so it cannot fall out of date with what the form actually sends.

Who else receives it

When you submit the form, what you typed is transmitted to Formspree ( formspree.io), a third-party form-handling service, which delivers it to us. They process it on our behalf and under their own privacy terms. We do not sell your information, and we do not share it with anyone else for marketing.

How long it is kept

Website enquiries are kept for 24 months so we can follow up, then deleted. This is separate from your clinical record, which is retained for the period required of a healthcare provider and described in our Notice of Privacy Practices.

Cookies and tracking

This site sets no cookies, runs no analytics script, and carries no advertising pixel. There is nothing here to opt out of. If that changes, this page changes with it.

Your choices

You can ask us what we hold about you, ask for a copy, ask us to correct it, or ask us to delete it. Email frontdesk@bayshorept.example and we will action it. You never have to use the form at all — the phone number at the top of every page reaches us directly.
